The long view
Ancient: Hippocrates and Galen, the four humours, ideas that lasted 1,500 years. Medieval: Islamic physicians like Ibn Sina preserved and extended Greek medicine while Europe's learning narrowed. Early modern: Vesalius on anatomy, Harvey on circulation. Modern: germ theory, anaesthetics, antiseptics, vaccination, antibiotics, the NHS in 1948, DNA and genetics.
What speeds change up, and what holds it back
Accelerators: war, government action, technology, communication, individual genius, chance. Brakes: entrenched authority, religion or tradition where it resisted, cost, and simple lack of a tool. The same factor can do both — war killed and also drove blood transfusion and plastic surgery forward.
Structuring the answer
Pick two or three factors, use precise examples with dates, and explain HOW each factor worked rather than listing it. Then judge which mattered most for the specific change in the question. 'Technology mattered because the microscope let Pasteur see what Galen could not' is an explanation; 'technology was important' is not.
Galen, working around 170 CE, dissected animals and got the human body substantially wrong. Because his work fitted Christian and Islamic ideas of a purposeful body, it became authority rather than evidence. Vesalius, dissecting humans in 1543, found over 200 errors — and was attacked for it. Fourteen centuries of continuity, then rapid change, driven by a factor you can name: the status of authority against observation.
